Office Visit & Surgery
Our goal is to make your experience at the Pediatric and Adolescent Urology Institute as pleasant, convenient and efficient as possible. Dr. Jeffrey Palmer and his staff are committed to providing you and your child with a positive experience.
Please assist us by preparing for the appointments and surgery by bringing some important information with you and preparing your child for the visit.
Preparation for Office Visit
In order to assist in registering and treating your child promptly, please do not forget to bring your current insurance information with you. We accept most types of health insurance. Please contact us for further details.
Previous medical records
Please bring with you all of your child’s previous medical records. Please do not have them mailed or faxed to us since many times they do not arrive in time for your child’s visit, which will delay your visit.
Previous radiographic studies
Please bring with you all of your child’s previous radiographic studies (ultrasound, VCUG, renal scan, CT scan, MRI). Please request the images on CD and the reports on paper. Please do not have the records mailed or faxed to us since many times they do not arrive in time for your child’s visit, which will delay your visit.
Center for Bladder Control
If your child has an appointment for bladder control issues, please complete the questionnaire on the Patient Portal prior to the visit. Please login using the user name and password provided to you when you register your child with our office. If you require a user name or password, please contact the office at (216) 292-2111
Also, please have your child drink prior to the visit and bring additional drinks with you in case your child requires additional hydration.
Accompanied by an Adult
Please arrange for an adult to accompany all children less than 18 years of age to each office visit. If the adult is not the child's legal guardian then please complete the attached form and have it brought the day of the appointment.
Preparation for Surgery
Our goal is to make your experience as pleasant, convenient and stress-free as possible. In order to assist in preparing you and your child for surgery, it is essential that the preoperative instructions are followed. For the safety of your child, he/she cannot eat or drink prior to surgery.
The time of surgery is usually based on the age of the child with the youngest child having the first appointment of the day and the oldest having the last appointment.
Dr. Palmer operates at several surgical facilities. Please confirm that the surgical facility is in-network with your insurance carrier.
